DISCHARGING A PATIENT ON WARFARIN

Please follow these guidelines when discharging a patient on warfarin:

I. Patients who were taking warfarin prior to admission:

  1. Determine who regularly manages the patient's warfarin therapy
  2. Call the provider currently managing anticoagulant therapy to re-refer the patient
  3. Schedule an appointment:
    1. in 1-3 days if patient and/or therapy are not stable
    2. in 3-7 days if patient and/or therapy is stable
  4. Fax the inpatient Anticoagulation Flow Sheet, as well as a complete discharge summary, to the provider currently managing anticoagulant therapy

II. Patients new to warfarin therapy:

  • Select an appropriate management strategy. Options include:
    • UWMC primary care provider
    • non-UWMC primary care provide
    • Outside anticoagulation clinic
    • UWMC Anticoagulation Clinics
  • If UWMC Anticoagulation Clinics is selected,
    call 206-598-4874
    • Leave the following information on voice mail or with the receptionist:
      1. patient name and hospital number
      2. current inpatient service and floor
      3. intern's or resident's name
    • Fax a UWMC Anticoagulation Clinic referral form to
      206-598-6217 after all information has been filled in
    • The UWMC Anticoagulation Clinic will contact you to obtain additional information and to determine an appropriate date and location for follow-up
  • If the primary care provider or an outside anticoagulation clinic is selected:
    • Call the follow-up provider or anticoagulation clinic to refer the patient
    • Schedule an appointment:
      1. in 1-3 days if therapy is not stable
      2. in 3-7 days if therapy is stable
    • Fax the inpatient Anticoagulation Flow Sheet to the follow-up provider or anticoagulation clinic, as well as a complete Discharge Summary
